Which countries will you be visiting and are you hoping to get tickets to a match or just take a stadium tour?  The process for buying tickets varies by league and even by team, and can be difficult if you live abroad or are not an official fan of the team.  

In Spain, Estadio Santiago Bernabéu in Madrid and Camp Nou in Barcelona are the ones to visit.  Both have good tours geared toward fans.  I believe it's easier to get tickets for Barcelona in advance, but once you're in Spain, you should be able to get tickets for either team.  Tickets are ridiculously hard to get when the two teams play one another, as they will on October 26th.

In Italy, Stadio San Siro "Giuseppe Meazza" (better known as San Siro) in Milan is an obvious choice. It's the home stadium of both of Milan's (former?) powerhouse teams.  In Germany, I'll second Laurel's rec.  In London, I'd visit Wembley Stadium if you're not attached to any particular team.
